This case study
will present a holistic approach to the care of a patient suffering with
constipation within the public setting. The assessment will be presented
through the understanding of the nursing process and will conclude with the
reflection of understanding the assessment and holistic care for the patient. The
patient chosen for this case study is Mrs Banks. For the purpose of this
assessment, Mrs Banks name has been changed to protect patient confidentiality.
Mrs Banks has given consent to use her condition for the focus of this

assessment process undertaken to assess Mrs Banks and the best plan of care for
her condition is the nursing process. Alrafo-LeFevre (2009) states that the
nursing process is made up of five different components. These five components
are assessment, diagnosis, outcome identification and planning, implementation
and evaluation. The assessment component is based on gathering evidence from a
variety of sources about Mrs Banks condition, as she has been suffering with
constipation for many months, there are general practitioner (GP) letters and
many medication leaflets in Mrs Banks home to initiate a holistic assessment of
her constipation. The assessment component of the nursing process also entails
validating and organising the collected evidence in order to get the best
possible outcome for Mrs Banks and her care she will receive. The second step
of the nursing process is diagnosis and involves analysis and synthesis to
further investigate the assessment. The nursing diagnosis is defined as the
nurse making a clinical judgement about an individual. (Alrafo-LeFevre, 2009)
The diagnosis the nurse uses in this component of the nursing process will
involve the nurse using critical-thinking and decision-making and the diagnosis
will give the nurse an idea of what nursing interventions need to be carried
out to best suit Mrs Banks and manage the treatment plan. If the nurse is
unsure of the diagnosis, they can carry out a risk diagnosis, an assessment
that could indicate a potential issue, but does not yet exist. (Alrafo-LeFevre,
2009) The third step of the nursing process is planning, this component of the
nursing process involves the formulation of the evidence and guidelines that
will be used to plan the care for the patient. When patient strengths and
weaknesses have been identified by using the nursing diagnosis, the nursing
professional can then begin planning the care for the patient. Within the plan
for the nursing care are the nursing diagnosis being prioritised, long-term and
short-term goals are put in place that are patient centred, interventions that
are specific to the patient are identified and developed and the whole plan of
care is recorded for the patient for nursing professionals to reflect and
reassess should this be required. (Alrafo-LeFevre, 2009) The fourth step of the
nursing process is implementation, this step entails implementing the care that
has been put in place by the nursing professional through carrying out the
first three steps of the nursing process. Involved in the implementation of the
care may be the delegation of specific tasks to other healthcare professionals
if needed. For example, delegating to unlicensed assistive professionals such
as care workers. Implementation involves skills such as assessing the patient
and their condition before, during and after the nursing intervention. Part of the implementation stage of the nursing process, nurses
must also record what has been carried out during their visit and report any
changes in the patient condition. (Alrafo-LeFevre, 2009) The fifth and final
step of the nursing process is evaluation. The nurse must gather information
from the visit to decide whether the patient goals have been met and if not,
the nursing professional will then need to decide if the nursing activities
will cease or carry on, in order for the status of the care plan to be
maintained, it is at this point, if the goal has not been met, that a full
reassessment will need to be carried out to make patient goals more realistic
and achievable.
